How did Andy die and in what episode?
Andy died in the Season 1 finale called Deja Vu All Over Again. Rodriguez the cop (also a demon) who was following the sisters around was trying to kill all of them off so that the Power of Three would be no more. In the morning, Phoebe has a premonition that Andy is going to die. So she tells Prue to tell him to stay away from the Manor. The first time Rodriguez tries he was able to kill one of them I think it was Phoebe. Prue was able to vanquish Rodriguez but he doesn't die. Insted he enables the help of the Devil's Sorcerer ~ Tempus(David Carradine, Kill Bill Vol 1&2) to turn back time, learn from his mistakes and try killing the sisters again. So the girls go through the same day once again and Phoebe has an odd feeling of Deja Vu, but she's the only one. She tells Prue to warn Andy again. Rodriguez comes back to the manor and this time he is able to get Piper and Phoebe, and then Prue vanquishes him. Rodriguez gets a third chance with the promise that he won't fail this time. This time Phoebe knows that something is up. She is able to tell Prue that Kit is about to knock over the salt shaker, and that their neighbors car was in a accident (and it wasn't Prue's like she thought), and then she tells Prue to hand her the newspaper where she gets the premonition of Andy dying. She tells Prue to go to Andy and tell him to stay away once again. The girls are ready for Rodriguez this time (or so they think). He is one step ahead he knocks out Prue. Andy realizes that something is wrong and heads into the manor from his car (where he was watching everything). He gets inside and shoots at Rodriguez, nothing happnes and Rodriguez hits him with an energy ball throwing him into the living room; where he lies dead. Prue in an unconscious state meets Andy on their swing, and she tried to tell him that they can make it so Rodriguez has to turn back time and he lives. But Andy says no, if that is done then they will all die at Rodriguez's hand. They have their final goodbye and Prue wakes up. They manage to get Rodriguez tied to a chair where they drill him on questions. He doesn't answer any of them and finally Prue decides to just let him go. But he's not going out without a fight and throws one last energy ball at the. Prue deflects it back at him and vanquishes him for good, along with weakening the Devil's Sorcerer ~ Tempus because of a spell Prue casted to accelerate time. She then goes and stands over Andy's lifeless body. They have a funeral for Andy and the episode ends with Prue closing the door to the manor.
